Insurance and Other Payment Plans
Suzanne is currently an “in-network” provider for some Colorado health insurance plans; however, she is not “in-network” for health insurance plans in California. If a health insurance plan is used, the copayments, deductibles, and/or coinsurance will be as determined by the insurance company.
For all others, the full fee for my services will be as indicated below. Suzanne can provide documentation so that clients can submit claims to other insurance companies for “out of network” benefit coverage.
Notice: For self-pay clients that are not going to be submitting claims to other insurance companies for “out of network” benefit coverage, a written good faith estimate (GFE) of expected charges will be provided and is also available upon request.
My Full Fee
Suzanne’s fee is $150 per 50-minute counseling session and $225 per 80-minute counseling session. It is recommended that couples therapy sessions last 80 minutes.
She accepts cash, check, and credit cards (including HSA cards).
